WORK PERMIT INFORMATION FOR STUDENTS AGES 14-17
Once a student has been hired, and is between the ages of 14 -17 years old, they will need to obtain a work permit application from their employer.
After a minor is promised employment, a work permit application must be filled out by the employer, physician (for students age 14 and 15 only), and signed by a parent/guardian and student.
The application can be found on Wareham Public Schools website. Hard copies are available at Wareham High School and Central office.
WORK PERMITS
Wareham High School issues Work Permits. Students between the ages of 14 and 17 years old may obtain an Employment Permit Application from the High School between 7:30 a.m. and 2:30 p.m. Monday through Friday, or download the application from the district website under Students and Parents > Student Work Applications.
The student should have the employer complete the Promise of Employment section of the application form; have it signed by the parent/guardian/custodian of the student; and sign it as the minor student. If a student is 14 or 15 years old, the student must have the Physician’s Certificate of Health portion of the application completed.
The student then brings the completed application along with a certified copy of one of the following: student’s birth certificate or passport to the Wareham High School Principal’s Office before obtaining an Employment Permit. There is no cost to the student to obtain a work permit.
A birth certificate is required if the student is a Wareham resident and has never attended Wareham Public Schools.
Upon receipt of the application, a work permit will be issued, and the student will give the signed permit to their new employer.
